Source Records and Provenance
The Patient Chart provides access to source records and associated provenance metadata to support validation, traceability, and interpretation of clinical data.
Source Record Access
In supported sections, each record may be linked to its underlying source.
- Source records may be represented as FHIR resources such as
DocumentReferenceandDiagnosticReport, or as unstructured documents - Selecting a record or document title opens the associated source record
- Source records may open in a modal or a new tab depending on content type
Where supported, users can:
- View the original document or FHIR resource associated with a record
- Review detailed metadata such as document title, author, and service date
- Download documents for external use
Source records provide the full clinical context for data displayed in structured views.
Provenance Metadata
Provenance metadata describes the origin and context of each record.
Common metadata elements include:
- Source or submitting organization
- Document or resource type and classification
- Service date and creation or transmission timestamps
- The record's source or import type, such as a network import or a client-submitted document
This information supports:
- Validation of clinical context
- Traceability to the originating system
- Audit and compliance workflows
Supported Sections
Source records and provenance metadata are typically available in sections that include structured or document-derived data, such as:
- Documents
- Labs and imaging
- Diagnoses / Problems
- Medications
- Allergies
- Vital signs
- Procedures
Availability may vary based on data source, record type, and system configuration.
Behavior
- Accessing source records does not initiate new upstream data retrieval
- Provenance reflects information available from the processed dataset
- Some records may not include full provenance details depending on source data
