Administration

Patient Chart Licensing Visibility: On the Network administration screen, the Network Stats Site Configuration report now provides the status of Patient Chart licensing at the tenant level.

Patient Chart

USCDI v3.1 API Expansion: Patient Chart APIs now support additional USCDI v3.1 data classes, including Clinical Tests, Laboratory specimen details, Health Insurance, Care Team Members, Encounter disposition, and new medical history record types (Goals, Health Concerns, Interventions, Indications), with new Related Person endpoints on the Patient endpoint and expanded demographic metadata for Gender and Ethnicity.

USCDI v3.1 Patient Chart Display: Demographic field labels were updated and Related Person demographics now display the person's name and Relationship values.

Local Data Deduplication and Source Categorization: The Patient Chart and chart APIs now use a client's local data to deduplicate against external data, classifying records by source: Internal (in the client's local system), External (from external networks), or Internal + External (in both). Records that exist in both sources appear once under the Internal + External category, local-only records are not displayed, and external-only records are marked with a Community Data icon. "Patient Not Found" Default Page: When a URL points to a patient record that does not exist, the Patient Chart now routes you to a Patient Not Found screen instead of rendering an empty or error state.

Filter by Provider Hidden for Tenants Without Providers: On the Documents view, the Filter by Provider control is hidden when the tenant has the Tenant without providers setting enabled.

Clinical Alerts

Clinical Alerts Enhancements: Clinical Alerts now retrieves the source document that triggered an alert and stores it on the patient's chart, so the underlying clinical record is available for review right away. To enable this feature, contact Health Gorilla.

ADT Notifications

QHIO ADT Integration: ADT data ingestion from QHIO sources has been expanded. Encounter records and demographic updates from participating QHIO facilities flow through event-driven ingestion and are enabled per tenant through the Patient360 — QHIO feature license.

Resolutions

QHIN ITI-39 Document Retrieval: Resolved QHIN ITI-39 (XCA-R Document Retrieve) failures that blocked document retrieval.

QHIN ITI-38 Whitespace: Fixed QHIN ITI-38 queries that failed when the $XDSDocumentEntryType parameter contained whitespace.

Patient Chart Date Filters: Fixed Patient Chart Procedures and Plan of Care queries that returned no results when fromDate and toDate filters were applied.

Deceased Patient Label: On the View Patient modal, restored the Deceased label and disabled the Retrieve data button for deceased patients.

CDA Validation: Updated CDA document generation to include the required raceCode and ethnicGroupCode elements.

Document Delete Function: Restored the Patient Chart API Document Delete operation for providers; previously the request returned 400 Bad Request with "Access denied: not a patient user."

Patient360 Cancel Retrieval: Restored the Cancel retrieval option in Patient360 to stop an in-progress retrieval.

FHIR Allergy Manifestation: Updated the FHIR Allergy converter to retain manifestation data when the source record lacks coding.

Medical History Encounter Duplicates: Corrected a duplicate-record issue that caused duplicate Encounter records in the Patient Chart.

Unsupported HTTP Method Response Code: Corrected REST endpoints to return 405 Method Not Allowed instead of 500 Internal Server Error for unsupported HTTP methods.

Diagnosis Status Filter: In the Patient Chart Viewer, fixed the Diagnosis status filter on the Diagnosis list so the Status column displays only the selected statuses.

Interoperability

Shareback Channel Attribution Improvements: Channel classification is now supported for medical history records, including identification of both internal and external sources and a new localImport channel for self-locator flows.

Administration

HIN Request Statistics in EHR Audit: Network Admin now links HIN request statistics to Patient360 audit transactions, allowing users to view source organization metrics from the transaction details.

Patient Chart

Medical Equipment UDI Data Support: Patient Chart now displays device UDI details in Medical Equipment, including the lot number, serial number, manufacturing date, expiration date, and identifier for USCDI compliance.

Medication Supply Display Enhancements: Patient Chart now displays medication supply details, including supply text, quantity, effective date, and status for USCDI compliance.

Encounter Disposition Display: Patient Chart now displays discharge disposition within the Encounters table and detail view for USCDI compliance.

Health Insurance Data Enhancements: Health insurance data support has been expanded across CDA processing, Patient Chart APIs, and the Patient Chart UI, including coverage type, subscriber ID, and payer ID for USCDI compliance.

Related Person Contact Information Enhancements: Patient Chart now displays related person contact details in the Demographics section to support USCDI requirements.

Procedures Default Filter Update: On the Patient Summary screen, the default filter for the Procedures section has been updated from Active to All, ensuring all procedure records are displayed by default.

• Labs and Imaging Action Panel Enhancements: Expanded action panel capabilities are now available for interacting with lab and imaging records.
• Vital Signs Enhancements: Vital sign records can now be added, edited, and removed.

Resolutions

Immunization Code Mapping for Missing CVX: Fixed Patient Chart immunization processing to populate a vaccine code when source documents include a translation code but a null CVX value.

Facility Status Update API: Resolved Network Admin REST API failures that prevented facility status updates.

Master Patient Endpoint Filtering: Fixed Network Admin master patient endpoint filtering to return results that match the provided search criteria.

Record Location Pagination: Corrected Patient Chart record location queries to return results when using pagination with a non-zero offset.

Immunizations API Update Behavior: Corrected minor issues in the Patient Chart Immunizations API affecting update validation and field persistence.

Patient Chart Source Document Navigation: Restored navigation for the “Source Document” link to correctly open the associated document.
